Dr.A.S.Sanjay, C/o. Vinayaka Clinic, No.35, Teachers Colony, Nagarbhavi, Bangalore is hereby authorized to extend his services as Authorized Medical Attendant to officers/officials residing in ULLAL area apart from the areas allocated in Circular No.1 dated 17-01-2012.
a) That he will charge consultation and injection fee at the rates prescribed under the Central Services(Medical Attendance) Rules,1944;
b) That he will be required to follow strictly the rules under the Central Services (Medical Attendance) Rules, 1944 and orders issued from time to time there under; and
c) That his nomination could b terminated even before the expiry of his term without assigning any reason or giving any notice.
